On a separate piece of paper or in space below, please tell us why the person you have nominated is a hero.  Be sure to  include any news articles, police reports, names of witnesses we may call to verify the heroic act.  Award decisions will be based largely on the information you provide, so please be as  detailed and specific as possible.

Nomination Guidelines Heroic event must be ongoing or have occurred during the period Jan. 1, 2008 - Dec. 31, 2008.  Nomination forms must be received by Thursday, Jan. 15, 2009 to be considered! Nominations may be made for both living and posthumous Heroes. Hero must live or work in one of the following towns, or the event must occur in one of the following towns:
